The Practice will accept patients from any part of the Isle of Lewis covered by postcode areas HS1 and HS2. Patients can be seen at Stornoway or Habost, whichever is most convenient.
You can register as a patient by visiting either surgery, Stornoway or Habost as appropriate, and completing the necessary registration forms. These forms are also available to download from our website, see the links below.
- You will be required to provide photo ID (passport, driving licence) when you register.
- You will be offered a new patient health check at the time of registration.
- You will be registered with the practice and not a specific doctor.
When making an appointment, should you wish to see a specific doctor you may request this at the time and we will do our best to accommodate you.
