We invite patients for an Annual Health Review during their birth month for:
- Chronic Diseases
- HRT and the contraceptive pill
We ask patients to complete these using eConsult, with the exception of Diabetic Annual Reviews which are carried out face-to-face.
BP and Weight Readings
A number of reviews require recent BP and weight readings. There is a Patient Health Monitor in our waiting room for patient use and the results can be included in the eConsult, or handed in at the front desk. Please ask for a token at our Reception desk.

HRT and Contraceptive Pill Annual Reviews
We invite patients to an annual review for HRT and the Contraceptive Pill in their birth month. Please note that you must include a recent weight and BP reading with these reviews.

If you are concerned about your health please do not wait to be called for a review. You can complete an eConsult here or phone either surgery and ask for an appointment.
Please do not use the eConsult system for urgent matters as we may not respond to your eConsult the same day it is submitted.
If you don’t receive an invitation during your birth month and you have a Chronic Disease please phone either surgery and ask for an appointment.
Invitation to attend
We invite patients using text messages. If we do not have your mobile number, we will make contact by phone or by letter.

Your completed eConsult
When your completed eConsult is received it will be reviewed by our Practice Nurses. If further information or additional tests are required our nurse will contact you.
Reviews in person
We understand that some patients prefer to have a review in person and we are happy to accommodate this. Patients from both our surgeries (Habost and Stornoway) can book appointments in the usual way by phone 01851 810689 or 701345 respectively.
