We ask patients to complete an eConsult in their birth month for COPD reviews.
Once the nurse has received the eConsult she will review this information. If she would like you to attend the surgery for any follow-up monitoring she will be in touch. Otherwise, your next review will be in your birth month the following year.
It is helpful for patients to complete a CAT questionnaire as part of the review. Once we receive your eConsult we will send you the questionnaire to complete and return by email.
If you attend the surgery for an asthma review please bring your inhalers and peak flow diary.
