mPower: Healthy and Connected Communities

You could be missing out on valuable information and tools that would support your health and well-being.

NHS Western Isles’ project mPower: Healthy and Connected Communities has been created to help local people take the steps needed to live well, safely and independently in their own homes.

It does this by linking them with services in their community and by helping people use technology to look after their own health and care. They can provide support to help you get to grips with online tools and IT, making sure you get the best of the services available to you.

You can pick up a Self-Referral form at Reception which allows you to refer to mPower without having to see your GP first.
